What the study showed, in plain terms

An early pilot reported lower glucose and HbA1c in both a randomized comparison and a separate add-on cohort. Similar changes to metformin in a small pilot do not establish therapeutic equivalence.

Key findings

  • In the randomized berberine arm, HbA1c changed from 9.5% to 7.5%; this is a within-arm change.
  • Transient gastrointestinal adverse effects were reported in 20 participants across berberine exposure.

What this study can and cannot tell us

Small samples, incomplete masking information and an uncontrolled add-on component limit inference. Gastrointestinal complaints occurred in about one third of berberine-treated participants.
