Alpha-ketoglutarate is the name of the molecule. Chemists call it AKG. It has one job everyone agrees on and several roles researchers are still working out.
Calcium is a mineral you already know. In Ca-AKG, it acts as a chemical partner that keeps the alpha-ketoglutarate stable inside a capsule. Once the capsule is dissolved by your stomach, calcium and alpha-ketoglutarate go their separate ways.
Some products call it Ca-AKG. Some call it CaAKG. Some call it calcium α-ketoglutarate with the Greek letter. All the same compound.
Where does your body get alpha-ketoglutarate?
You make it. All the time. Without needing to eat any.
Alpha-ketoglutarate sits in the middle of a chemical loop called the Krebs cycle. This is the loop your cells use to turn food into energy. Every time your cells burn glucose, fatty acids, or amino acids for fuel, they route the process through the Krebs cycle — and alpha-ketoglutarate is one of the stops on that loop.

What is Ca-AKG supposed to do?
Three roles come up in almost every scientific paper on AKG.
Energy. AKG is a required step in how cells make ATP, the currency your body spends every time it moves, thinks, or repairs itself.
Building blocks. AKG is a precursor for several amino acids, and it plays a role in the machinery that makes collagen — the protein that holds your skin, tendons, and connective tissue together.
Gene regulation. A family of enzymes that switch genes on and off across your body needs AKG to do their job. When AKG levels drop, some of that regulation starts to drift. The idea that this drift contributes to how we age is one of the reasons AKG became interesting to longevity researchers.

Why do people take it as a supplement?
Mostly because of a 2020 mouse study.
Researchers at the Buck Institute for Research on Aging fed calcium alpha-ketoglutarate to middle-aged mice. Female mice lived meaningfully longer. Both male and female mice spent much less of their lives being frail. The paper called this "compressed morbidity" — more healthy time, less sick time [2]. That paper is why every longevity supplement brand you have ever heard of started stocking Ca-AKG.
The human story is more cautious. A 2022 review in Trends in Endocrinology and Metabolism looked at the wider human AKG literature and concluded the compound was worth serious clinical study — but that solid human evidence for anti-ageing effects was not yet in [3]. A widely-shared 2021 observational study reported that people using a delayed-release Ca-AKG product showed an average 8-year reduction in DNA methylation age [4]. A larger 2026 analysis of 4,260 people replicated that signal for the delayed-release form — but found the standard immediate-release Ca-AKG effect was much smaller and not statistically significant [5].
Two properly designed randomised human trials are in the field right now. Neither has reported yet.
Is calcium alpha-ketoglutarate the same as calcium?
No. This one confuses a lot of people.
A typical 1,000 mg dose of Ca-AKG contains roughly 200 mg of actual calcium. That is meaningful — about 15 to 20 percent of what most adults need in a day. If you already take a calcium supplement, or eat a very calcium-rich diet, add Ca-AKG's calcium into your total.
But the reason anyone takes Ca-AKG for longevity is the alpha-ketoglutarate half. The calcium is along for the ride.
If you are looking for a calcium supplement specifically, Ca-AKG is not the most efficient way to get it. Ordinary calcium carbonate or calcium citrate delivers more calcium per capsule at a much lower price.
Is Ca-AKG good for your kidneys?
Two answers, both true.
The history is reassuring. Calcium alpha-ketoglutarate has been used in kidney medicine for decades — as a supportive treatment for patients on haemodialysis and as a phosphate-binding agent. A 2017 review in Archivum Immunologiae et Therapiae Experimentalis catalogues this history in detail [6]. The compound has a long safety record in people with kidney disease under medical supervision.
The practical caution is about the calcium. If you have a personal or family history of kidney stones, extra calcium from any source is worth discussing with your doctor before you start. This is not a Ca-AKG-specific warning — it applies to any calcium-containing supplement.

Does Ca-AKG actually reverse ageing?
The honest answer is: probably not, in the way the marketing implies.
What has been shown: middle-aged mice fed Ca-AKG live longer and stay healthier for longer. Middle-aged people who take a delayed-release Ca-AKG product show slightly lower biological age on one type of laboratory test. Neither of these adds up to "reversing ageing."
Nobody has shown that Ca-AKG lowers your risk of heart disease. Or dementia. Or cancer. Or any other age-related condition. Nobody has shown it extends human lifespan. Two randomised trials will move this conversation in the next 12 to 18 months. Until they read out, "may support healthy ageing" is roughly the strongest claim the evidence supports.
What Ca-AKG is not
Three quick misconceptions worth clearing up.
Ca-AKG is not a peptide. Peptides are short chains of amino acids. Ca-AKG is a small molecule and a mineral salt. If you see it described as a "peptide" on a retailer site, that is wrong.
Ca-AKG is not the same as AAKG. AAKG (arginine alpha-ketoglutarate) is a different salt of the same molecule, marketed mainly for exercise performance rather than longevity. Ca-AKG is not a medicine. It is a food supplement, regulated as such. That means no health authority has approved it to treat, cure, or prevent any specific condition — and the quality of what you actually get in the capsule depends entirely on the manufacturer.
Should you take Ca-AKG?
Depends on what you are trying to achieve, how you feel about acting on early evidence, and what you can afford.
If you are comfortable acting on the current evidence base — encouraging animal data, promising but early human observational data, two ongoing trials — Ca-AKG is a plausible longevity supplement to consider. The safety record at typical doses is acceptable. The published human trials so far have not shown significant adverse effects.
If you would rather wait for the trials to report, that is a reasonable position too. Waiting 12 to 18 months for the ABLE trial and the Chinese trial to read out will substantially change what anyone can honestly say about Ca-AKG's effect in humans.
